Forms
A form is a capture block you build in the app: every submission becomes a lead in your funnel, with the filled fields saved onto the contact. It is how you trade a page or some traffic for contact details inside Clickmax.
Role and plan
- Creating and editing forms is open to any workspace user with CRM access — no restricted role.
How a form works
Each form has a name, a title and a set of fields. When you publish it, you can attach it to a funnel, a page or a custom domain, and each submission produces a lead.
Every field is mapped to a destination on the contact — either a standard column (name, email, phone) or a custom field in your CRM. What the visitor types lands in the right place, with no rework.
Form or quiz
A form and a quiz are the same kind of capture resource, split by how they ask: a form is a direct, single-step capture; a quiz is multi-step, with scoring and a result. If you need questions in sequence and segmentation by answer, use a quiz.
Where to publish
- Inside a funnel, as the capture step.
- On a page built in the editor.
- Embedded in an external site — in that case use the forms Browser SDK, which covers embedding, modals, programmatic use and unstyled forms (reference in Portuguese).

If it didn't work
- You cannot create or edit the form: check your access in no permission.
- Leads do not appear in the funnel, or you hit the plan ceiling: see the contact limit (Portuguese).
